It is time for the annual survey of people with bleeding disorders and chronic hepatitis C in New Zealand.
Every day at HFNZ we strive to provide the best service possible for every person with a bleeding disorder in New Zealand and their families. This is especially true of people who were infected with hepatitis C. One way of doing this is to actively seek feedback about you and your experience with hepatitis C.
Not only is this information important for us to continue to receive funding from the Ministry of Health to specifically support members with bleeding disorders and hepatitis C, but it also gives us a chance to listen to your thoughts, identify areas we need to work on and make improvements.
The survey will only take approximately 15 minutes to complete. Your personal information will remain confidential to HFNZ. Even if you have cleared HCV in the previous 12 months, please complete and return the survey (for the last time).

- Complete the survey in person or over the phone with your Outreach Worker on 0508 322 867. If you need help with any questions or are not able to complete the survey due to a disability, contact your Outreach Worker who will be happy to assist you. They will tick the boxes and write the comments you want to give on your behalf.
