2014 Hepatitis C Survey

Your Opinion is Important!

It is time for the annual survey of people with bleeding disorders and chronic hepatitis C in New Zealand.

Every day at HNZ we strive to provide the best service possible for every person with a bleeding disorder in New Zealand and their families. This is especially true of people who were infected with hepatitis C. One way of doing this is to actively seek feedback about you and your experience with hepatitis C.


Not only is this information important for us to continue to receive funding from the Ministry of Health to specifically support members with bleeding disorders and hepatitis C, but it also gives us a chance to listen to your thoughts, identify areas we need to work on and make improvements.

The survey will only take approximately 15 minutes to complete. Your personal information will remain confidential to HNZ. Even if you have cleared HCV in the previous 12 months, please complete and return the survey (for the last time).

We know your time and energy are valuable. As a token of our appreciation when you return your completed survey you will receive your choice of $20 gift voucher from either Pak n Save voucher or the Warehouse. You can get yourself a healthy snack or maybe something to read to relax with.

You can return the survey in 4 ways:

  1. Complete the survey online. Click to start survey now.
  2. Complete the hardcopy version that will be mailed to you in the post and return to Freepost 188174, HNZ, PO Box 7647, Sydenham, Christchurch 8240, Attn: Chantal
  3. Scan your hardcopy version and email to: info@haemophilia.org.nz
  4. Complete the survey in person or over the phone with your Outreach Worker on 0508 322 867. If you need help with any questions or are not able to complete the survey due to a disability, contact your Outreach Worker who will be happy to assist you. They will tick the boxes and write the comments you want to give on your behalf.
